- DEPARTMENT NAME: Name of the requesting department.
- DATE OF REQUEST: Date of preparation.
- FINANCIAL OUC CODE: The six digit numerical designator of the requesting department.
- MAP TO: The five digit project reference number from the parent project.
- PURPOSE CODE: The three digit numerical identifier designating the function of the sponsored award. The following are the most commonly used purpose codes:
- 110 Organized Research
- 122 Agricultural Research Service (CALS only)
- 131 Agricultural Extension Service (CALS only)
- 142 Community Services
- 152 General Academic Support
- 170 Institutional Support
- 401 Education Agreements
- 402 International Agreements
- PRIOR YEAR PROJECT NUMBER: If this is a continuing grant, such as NSF, PHS, or a Center project the prior year's project number is entered here.
- AMOUNT OF AWARD: Amount should include direct and F&A costs. If a project segment is requested: 1) ensure funds are available from the parent; 2) ensure total budget does not exceed award $.
- PHASE REQUIRED: This designator is used by those colleges/units that assign phases to their projects. Managed by the College only.
- PHASE NUMBER: Enter the five-digit phase here. Managed by the College only.
- COST SHARING COMMITMENT: Completed only if cost sharing is required.
- SPONSOR: The Agency providing funding for the project.
- CONTRACT, GRANT or AGREEMENT NUMBER: The agency identifier normally found on the face of the award document.
- PRINCIPAL INVESTIGATOR NAME: Name(s) of the project director. The full name should be shown. If there are multiple investigators, each person's name should be listed.
- STATEMENT OF FINANCIAL RESPONSIBILITY: Circle College or Department Responsibility.
