Accommodations and services cannot be guaranteed if students choose not to follow the established procedures in a timely manner. All accommodations and services are individualized and based upon the student's documentation. Accommodations and services can be revisited as needed, but they are not retroactive and cannot be guaranteed if procedures are not followed with reasonable, advanced notice. You should make an appointment with your service provider to request changes or additions. At that time, service providers can also share information regarding other possible campus and community resources.

How to Receive Accommodations and/or Services
Once registered with the DSO, each semester you must:

Complete and submit a DSO Request for Accommodations form or a Student Request for Services (Non ADA/504) form. These forms can be completed as soon as registration is complete. It is highly recommended that you make your request at the beginning of each semester or sooner when indicated. Your DSO Service Provider will send a letter by email to instructors. You will be copied on this email.
Contact each instructor (by email, telephone, or in person), to obtain course information and to arrange accommodations and/or services. This contact usually occurs after the instructor has received the letter from the DSO. IF YOU DO NOT CONTACT YOUR INSTRUCTOR, HE/SHE IS UNDER NO OBLIGATION TO PROVIDE ACCOMMODATIONS AND/OR SERVICES.

You also need to:

Maintain communication with your instructors during the semester regarding accommodations and/or services.
Contact the instructor and the DSO immediately if any significant course changes occur (drop, add, etc.).
Inform the instructor and the DSO service provider immediately if any problems regarding accommodations and/or services occur.
