Physical Disabilities

Documentation for Physical disabilities must be submitted by a licensed diagnostician, such as a physician, surgeon, physical therapist, occupational therapist, neuropsychologists, speech therapist or another relevantly trained medical doctor or provider who has undergone comprehensive training in the assessment of physical disabilities.

Your doctor or diagnostician must submit:

A fully completed Documentation of Disability Form and a copy of your most recent evaluation. This evaluation must be current unless the impairment is documented as stable and not capable of improvement. The evaluation should be a complete assessment of your impairment and your current level of functioning in an academic environment. It must include:
- A statement and history of diagnosis.
- The procedures used to assess your impairment.
- The tests used with dates administered.

Your diagnostician must list the substantial functional limitations and provide:

A full description of the resulting current difficulties and functional limitations.
The severity of each limitation and the impact and of these limitations on learning and functioning in this academic environment.

Whenever possible, your diagnostician should include:

Any recommendations regarding the academic accommodations and/or services needed for access.
A clear and convincing rationale for each requested accommodation and/or service based on the degree of impact of the impairment and the resulting functional limitations.
The history of accommodations provided in other academic settings.
