
The DSO provides testing space and delivers testing accommodations and services as a service to faculty as our space and staff permit. Requests should be made early in the semester for all exams students will take at the DSO during the semester. Testing appointments are first come, first served and are made only when all information is complete. Missing information will delay scheduling of the test. We do not guarantee testing on the same day or during the same week that we receive the request. The DSO is not involved when the instructor and the student decide that the test will be administered somewhere other than the DSO.
The DSO is not responsible for the integrity/security of any tests not administered in the DSO.
Before submitting a Modified Testing Agreement, the student must have already:
DSO provides testing accommodations and services when:
- The instructor’s schedule does not allow for extended time
- The instructor does not have access to a low distraction testing room
- The student's schedule conflicts with the instructor’s schedule
DSO testing environments are:
- Large group setting for extended time only
- Separate smaller group setting with a reduced number of distractions but not distraction free
- Individual room with a reduced number of distractions but not distraction free
Instructor
- After receipt of the DSO semester letter, the student should contact you to discuss and arrange testing accommodations or services.
- If you are not contacted by a student to discuss and make arrangements for testing accommodations and services, you are under no obligation to provide testing accommodations or services. You have the right to advanced notice and sufficient time to set up testing accommodations and services.
- If testing at the DSO, a Modified Testing Agreement form should be completed with the student. The MTA can be completed and submitted on-line, faxed to 919-882-0954, or sent by email to dso_testing@ncsu.edu.
- Make sure the form is fully completed. Provide the first and last names of the student and the number and section of your course on the form. List specific times and dates the student is to test at DSO bearing in mind that the dates may be different from the class if the student has a course conflict.
- Tests may be faxed to 919-882-0954; sent by email to dso_testing@ncsu.edu; or delivered by instructor, department designee, or the student. When sending a test, please provide the student’s name. If you provide more than one test version, please give instructions.
- When tests are completed, they will be returned to the faculty by email unless otherwise noted (to be returned via fax or picked up by instructor/department designee) on Modified Testing Agreement form.
- If a test date or time is changed, send an email to dso_testing@ncsu.edu with the updates and instructions. Students are not authorized to make test changes.
Student
- Contact each instructor to discuss and arrange testing accommodations and services. Contact usually occurs after the instructor has received the DSO letter. All testing accommodations and services are based on the materials and time allotted to the class as well as essential academic requirements.
- If you do not contact your instructors to discuss and make arrangements testing accommodations and/or services could be delayed. Instructors have the right to advanced notice and sufficient time to set up testing accommodations and services.
- Submit a Modified Testing Agreement form. If the instructor is unable to provide testing accommodations and Services in the academic department, you may test at the DSO.
- To request testing at the DSO, you must:
- Obtain copies of the Modified Testing Agreement form.
- Complete the form with your instructors. Incomplete forms will not be accepted.
- For scheduling reasons, list all of your exams and quizzes on one form.
- Discuss and resolve any testing conflicts with your instructors early in the semester and well in advance of the tests.
- Make a copy of each form so that you will know your test schedule, what time you need to report to the DSO for each test, your accommodations and services, and the materials allotted by different instructors. The DSO does not make reminder calls for students.
- Turn in each Modified Testing Agreement to the DSO front desk at least one week before the first scheduled test on the form. Testing space is severely limited.
- Submit any changes or updates to the DSO in writing from the instructor at least one week in advance. Changes and updates may be accepted with less notice, but cannot be guaranteed.
- Remember to schedule final exams as soon as possible. The DSO has a limited number of spaces and many, many, many testers. The DSO will proctor late exams the day after the last day of final exams, but not after that time.
- Maintain Communication
- Remind instructors the class period before each test or exam about testing at the DSO so they can prepare your accommodations and services.
- Arrange with the instructor for all testing materials to be brought to the DSO. The DSO does not provide testing materials.
- Students testing at the DSO do not have access to their instructors. Discuss with each instructor any anticipated need to make contact during an exam and make advanced arrangements for this contact.
- Inform the instructor and the DSO service provider immediately if any problems regarding testing accommodations and services occur.
- Contact the instructor and the DSO immediately if you drop a course for which you requested or have been receiving testing accommodations and services.
- Day of the Test
- Report to the DSO at the scheduled time. Testers who arrive late will be docked time just as would students in the classroom.
- Do not bring non-testing materials to the DSO, including, but not limited to, cell phones, pagers, PDA's, pocket books or book bags. If you do bring these items, the DSO will hold them but will not be held responsible for their safety.

- Provide testing space when given at least one week of advanced notice, a completed Modified Testing Agreement form, and when space is available.
