Documentation for Attention Deficit Hyperactivity Disorder must be submitted by a licensed professional, such as a clinical psychologist, neuropsychologist, psychiatrist, or other relevantly trained medical doctor who has undergone comprehensive training and has relevant experience in differential diagnosis and the full range of psychiatric disorders.
Your doctor or diagnostician must submit:
- A fully completed Attention Deficit Hyperactivity Disorder Verification Form and copies of your documentation (current within the past year), i.e., test results and diagnostic evaluations, cognitive and achievement measures/deficits administered or techniques used with dates, and results including subtest scores and/or percentiles
- A signed report on letterhead that includes all the information requested on the form. The report must include information about your doctor’s degrees, license or certification, and area of specialization.
Your documentation should include a brief, relevant history of the condition or disability, and should answer the following questions:
- What is the current diagnosis and when was it made?
- What is the current plan of treatment? How often is the student scheduled for treatment? When was the student’s last visit?
- Is the student on medication? If so, what are the side effects? Will the medication or treatment have any effect on the student’s functioning in an academic environment?
Your doctor or diagnostician must list the substantial functional limitations for each condition or disability.
- Which major life activities are affected by the student’s condition or disability (breathing, caring for oneself, hearing, learning, seeing, walking, performing manual tasks, etc.)?
- How does the condition or disability affect the student’s daily living activities as they relate to an academic environment?
- What is the frequency, duration, and intensity of the limitations?
Your doctor or diagnostician should include a list of recommended accommodations for each condition or disability, and include a rationale for each accommodation.
