
Documentation for Autism Spectrum Disorders must be submitted by a licensed diagnostician, such as a psychiatrist, clinical psychologist, or neuropsychologist.
Your doctor or diagnostician must submit:
- A fully completed Documentation of Disability Form and a written report of a current evaluation (within three years). The evaluation should be a complete assessment of your impairment and your current level of functioning in an academic environment. It must include:
- A statement and history of diagnosis.
- The procedures used to assess your impairment.
- The test instruments used with dates administered and results including all subtest, standard, and percentile scores.
Your diagnostician must list the substantial functional limitations for each condition or disability and provide:
- A full description of the resulting current difficulties and functional limitations.
- The severity of each limitation and the impact of these limitations on learning and functioning in this academic environment.
Whenever possible your diagnostician should include:
- Any recommendations regarding the academic accommodations and/or services needed for access.
- A clear and convincing rationale for each requested accommodation and/or service based on the degree of impact of the impairment and the resulting functional limitations.
- The history of accommodations provided in other academic settings.
