Office for Equal Opportunity
Discrimination and Harassment Prevention and Response Training Registration Form

To all NC State Employees:

In June 2007, NC State approved a regulation that requires every employee to participate in a program targeted at preventing and responding to discrimination and harassment.

The Office for Equal Opportunity provides this training. There are several options for completing the training requirement, including:

  1. Completing the Online Discrimination and Harassment Prevention and Response Training;
  2. Registering for an open enrollment session below; or
  3. Scheduling a training session for your department or unit by contacting the Office for Equal Opportunity at 919-513-1234.

The training, in any format, lasts approximately one hour.

You may have already fulfilled the training requirement by attending New Employee Orientation on campus within the past two years, completing the Equal Opportunity Institute, participating in the Supervisory Series, or attending a program coordinated by your department. If you think you have already fulfilled the requirement, please complete the section of this form titled "Previously Fulfilled Requirement."

In addition, supervisors are required to complete an additional supervisor-specific training program. The supervisor program is separate from the program required for all employees, and it should be completed only after a supervisor has participated in the general session. As of now, supervisor-specific sections are available through open enrollment (below) and departmental scheduling through the Office for Equal Opportunity.

NC State's Discrimination and Harassment Prevention and Response Training regulation requires that all current employees complete this training. Please meet your requirement as soon as possible!

Thank you for your help in eliminating discrimination and harassment at NC State.

Note: You may alternately use the secure version of this form.
