Complete the fields below regarding your appointee(s). Please note committee composition before submitting nominations. (Some will have two (2) nominations).

The selection committee is comprised of the following:

One (1) member of the Academy of Outstanding Teachers appointed by the Dean of each college, (10 members total)

Chair of the Evaluation of Teaching Committee, or member of the University Standing Committee on the Evaluation of Teaching appointed upon the recommendation by the chair

Chair of the Faculty, or one (1) faculty senator upon the recommendation by the chair

Two (2) graduate students appointed by the president of the Graduate Student Association

President of the Student Body and President of the Student Senate preferably, or two (2) undergraduate students appointed upon the joint recommendation of the presidents
