awards :: Board of Governors Award for Excellence - Submission Packet
Submit one paper copy and one CD of the entire submission packet outlined below. This includes a high-resolution jpg of the photograph. Do not send the entire packet as a PDF file. (NOTE: will be the same as an updated ADUP and Outstanding Teacher submission)

- Prepare one packet of original submission documents and a copy on a CD of original submission packet.
- Do not convert the combined packet as a single PDF file. Only items a, h, and i may be converted into individual PDF files.
- Create a table of contents page for the front of the original paper submission packet.
- Print on front and back of pages except for the nomination form. NOTE: Front/back page printing counts as 2 pages.
- Comply with page limits for sections as noted. Exceeding limits will disqualify nomination.
- Use a colored page between sections (a, b, c, etc). Do NOT use tabs.
- Do not submit paper copy in a binder/notebook; binder-clip the paper copy.
Submissions must be delivered to Virginia Howell, 415 Clark Hall, (adjacent to Clark Dining Hall), by 2:00 p.m. on Thursday, December 10, 2009.
