awards :: Outstanding Teacher Award - Revised Nominations Packet (see changes to sections c, d, g and h)

Nomination Packet

Submit the paper original and one CD of the entire submission packet outlined below.  This includes a high-resolution jpg of the photograph.  Do not combine the entire packet as a PDF file for the CD.

a. Completed OT Nomination Form signed by the dean or designee
b. Current curriculum vitae or resume
c. Brief statement articulating the candidate’s contributions to the teaching mission of the university, not to exceed 2 pages (revised)
d. Samples of course materials (such as excerpts from syllabi, assignments, projects, exams, student portfolios, or other evidence of student learning outcome) or documentation of teaching-related activities such as leadership relatesd to teaching and learning or continuing professional development (workshops, institutes, etc.) 4-8 pages (revised)
e. Tabulation of student evaluation ratings for each course taught the two years prior to the year of  nomination as well as departmental average and range
f. Summaries of two recent peer evaluations (if appropriate for nominee's appointment)
g. Letters of support from two peers, 1 page each (revised)
h. Letters of support from two students, 1 page each (revised)
i. 500 word professional bio, suitable for numerous publications
j. One high resolution 5x7 color photograph suitable for publication
  • Create a table of contents page for the front of the original paper submission packet.
  • Prepare one packet of original submission documents and one copy on a CD of original submission packet.
  • For the CD, do not convert the combined packet as a single PDF file. Items a, g, h, may be converted into individual PDF files for the CD. The jpg of the color photo should be 300dpi.
  • Comply with page limits for sections as noted.  Exceeding limits will disqualify nomination.
  • Use a colored page between sections (a, b, c, etc).  Do NOT use tabs.
  • Do not submit paper original in a binder/notebook; do not 3-hole punch; binder-clip the paper original.

Submissions must be delivered to Virginia Howell, 415 Clark Hall, (adjacent to Clark Dining Hall), by 2:00 p.m. on Friday, January 29, 2010.
