Q.My student's health record is not up to date. What can I do?

A. Included in your student's admissions package is the Student Health Services health and immunization requirements for each age group and college. Student Health Services recommends consulting with your family physician to be sure that all requirements are met before the start of school since students can not be treated until all medical forms are complete. For more information, contact the Immunizations and Medical Records department at Student Health Services at (919) 515-7233. To obtain a medical records and immunizations form, you may call (919) 513-3278 or visit or write: NCSU Student Health Center Campus Box 7304 2815 Cates Avenue Raleigh, NC 27695 .

Q.Does NC State offer student health insurance?

A. NC State strongly encourages each student to own a comprehensive medical insurance policy. A brochure describing the NCSU Student Insurance program is available by calling (919) 515-2563 or stopping by the Student Health Center located at the corner of Cates Avenue and Dan Allen Drive.
