Final Evaluations

J-1 Student Interns and their supervisors are required to complete in a final evaluation at the end of the internship period. The evaluation should incorporate both the learning outcomes and assessment tools described in the Form DS-7002 as well as any company assessment processes.

Below are the forms that the intern and his/her supervisor need to fill out an submit in the final week of the internship. Failure to complete the evaluations may jeopardize the intern's J-1 status and future eligibility to participate in the GTI internship program.
