Submit a Work Request
If this is an emergency during business hours (Monday - Friday, 8 a.m. to 5 p.m.), please contact us by phone at (919) 515-3040. After hours, please contact your 24-hour service desk.
Greek Village residents should contact the Greek Life Chapter Resident Advisor on-call at (919) 622-8175 for all after-hours emergencies.
If this is not an emergency, please follow the directions below:
- Click on the Submit a Request button above to enter a request.
- Enter the full name of the person to be contacted regarding this request. (Note: If you have done a previous request your requestor information may already be populated.)
- Enter the phone number to be contacted regarding this request.
- Enter the e-mail address to be contacted regarding this request.
- Select the Facility area where the work is to be performed from the drop down menu by clicking on the down arrow. If you do not see the proper Site in the list contact your 24 hour service desk.
- Select the Building where the work is to be performed from the drop down menu by clicking on the down arrow. If you do not see the proper Building in the list contact your TMA System Administrator.
- Select the Location Asset where the work is to be performed from the drop down menu by clicking on the down arrow. If you do not see the proper Location Asset in the list contact your 24 hour service desk.
- Select the Request Type Description of iServiceDesk Request from the drop down menu by clicking on the down arrow.
- Give a full description of your request. (Note: If you do not provide sufficient information for your request it may be rejected.)
- Set the Notify Me option if you would like to receive email updates regarding your request.
- Click the Submit button. You will then receive your Request Number. Please record this number for your records. If you selected to be notified, you will also receive an e-mail letting you know your request has been received. (Note: If you do not get a request number you have not successfully submitted a request.)
Please submit one request per task, per area, per form. If multiple requests are received per form they may be rejected.
Telecommunications issues (such as phone, internet, or cable TV) should be submitted directly to Communications Technology at (919) 515-7099. All housekeeping issues should be reported directly to the 24-hour service desk.
