On the next page, please list factors which may have had, are currently having, or may have in the future, a significant influence on the center, its personnel, and/or its development.

 

If the influence was/is positive, the factor is identified as a facilitator.

 

If the influence was/is negative, the factor is identified as a barrier.

 

Please use the following code for each factor:

 

P – (past). The factor had influenced the Center in the past.

 

C – (current). The factor is currently influencing the Center
