NBC 17 Health and Fitness Fair
Volunteer Form

The NC-ACS Section will again participate in the NBC 17 Health and Fitness Fair. We will have a booth similar to the one at the State Fair. All members of the Local Section (YCC, SCC, student affiliates and any others) are encouraged to participate. Volunteers will be paired with other volunteers who have worked before either at the Health Fair or the State Fair.

• When: May 17-18, 2008
• Where: Graham Building at the NC State Fairgrounds, Raleigh, NC. See the NBC 17 Health and Fitness Fair website for information.
• About our Booth: Two volunteers are needed for each 3.5 hour time slot. At the booth, volunteers will help children do hands-on activities, perform experiments for all ages and answer questions.
• Live Stage Shows: To be arranged.
• Tickets & Parking: none required.
• Company Logos: If employees from your company volunteer, we will be happy to display your company logo as part of a PowerPoint presentation given during the stage shows.

We are looking for your preferences regarding the 3 1/2-hour block(s) of time from each volunteer. Please feel free to volunteer in pairs.

To assist with scheduling, we would like to receive (1) the maximum number of time slots you would be willing to work: and (2) the preferred time slots. We cannot guarantee that you will get all time slots, but we will confirm the assigned times and the alternate times well in advance of the opening of the Health Fair.
