Report of Medical History and Immunization Record
All international students are required to have their Report of Medical History and Immunization Record form completed,
signed by their doctor, and submitted to Student Health Services immediately upon acceptance to ensure they are in compliance
with North Carolina Immunization law.
All records must be translated into English. You must submit your Health History and Immunization Form before you arrive at NC State University through the
Healthweb system.
Please Note: Student Health Services requires that the Tuberculosis (TB) Test be administered after arriving in the United States. Student Health Services
is happy to schedule appointments for students needing this test. You can contact Student Health Services to make an individual appointment. You can also make a reservation through OIS online.
You must submit records of required immunizations online at https://healthweb.ncsu.edu but you are also
required to submit official copies of your
immunization documentation (in English) in person or via e-mail to Student Health Services. Check your secure messages at
healthweb.ncsu.edu for updates and communication from the Immunization Office.
