J-1 Student
Academic Training Recommendation Online Form

(For UG Advisors or DGPs and Committee Chairs only)

J-1 students MUST obtain employment authorization in SEVIS before beginning all types and instances of employment including AT. OIS advisors must certify in SEVIS that the proposed employment is related to the student's field of study, original research goals, and/or educational objectives. For us to do this, we kindly ask the student's DGP (some DGPs will forward the request to the Committee Chair) to review the offer letter to see if it meets these conditions and to recommend the AT (or not) to OIS. We appreciate your taking a few minutes to answer the following questions below.

J-1 students sponsored by NC State who wish to apply for AT should first submit a request in the OIS HelpDesk system. They should then ask you to complete this on-line recommendation form. You may also print out a blank copy and fill it in by hand if you wish or if you have trouble submitting the online version. The OIS advisor will collect the student's request, a copy of the offer letter, and your online recommendation then enter the data into SEVIS. When a new DS-2019 with the new Employment (and new end date) is returned, OIS will contact the student to pick up the new DS-2019 and employment authorization. Within a few days of receipt of all documents (the Student's request in the OIS HelpDesk, a copy of hte offer letter, and the online Advisor's Recommendation, the student should receive a response from the OIS HelpDesk system regarding the AT request. After the information has been uploaded into the SEVIS system, we will generate a new DS-2019 and contact the student to pick it up.

If you experience difficulty submitting this request online, you may scan it and submit it through the OIS HelpDesk system (preferred), fax it to OIS (515-1402), or bring/send a hard copy to OIS.
