SEVIS Enrollment Form for International Applicants to NC State University
 

F-1 & J-1 Student Applicants to NC State:

This Sevis Enrollment Form is only for international applicants who have been offered admission to a degree or nondegree program at NC State and to notify OIS of your intent and request to be entered into SEVIS. The information asked below is required for all F-1 and J-1 students at NC State - as well as any dependent family members in (or who wish to be in) F-2 or J-2 status - who are offered admission to or are enrolled at NC State. This information is also requested on the Certificate of Financial Responsibility (CFR) and if you have already submitted a complete CFR for your term of admission, this form is not necessary. Neither does this form replace the CFR. It is mainly used for applicants to update or correct the information submitted on an earlier CFR or by those students who may have received a Graduate Student TA or RA (and OIS accepted the Award Letter in lieu of a CFR).
 

If, after reviewing the information you submit with this on-line Sevis Enrollment Form, OIS can complete the immigration part of your admission process, we will notify the Admissions office of your admission clearance and will submit the information in SEVIS. After receiving the admission packet from the admissions office, we will combine the formal admission letter, the visa certificate (I-20 or DS-2019) and some very important pre-arrival information and mail it to you at the address we have on record (or you indicate). (The packet may be sent to your department or a sponsor if prior arrangements have been made.) Please note: we cannot express mail the packet to you at our expense and our express mail company will not permit us to bill the recipient. If you wish to have a local contact in Raleigh pick up your packet and express mail it to you, please complete the pick-up section below.
