INSTRUCTIONS ON COMPLETING THE
CERTIFICATE OF FINANCIAL RESPONSIBILITY (CFR)

INTRODUCTION: Signatures and bank documents must be current (less than 6 months from the date of intended enrollment; i.e. dated no earlier than February 1 for Fall admission and no earlier than July 1 for Spring admission). All documents must be original- no faxes or photocopies will be accepted. A Form I-20 or Form DS-2019 cannot be issued until this form has been approved by OISSS (or the Graduate School ONLY if applicant is overseas planning to apply for an initial F-1 visa). If you already have a passport, please also attach a copy of your passport identification page, so we can be sure the name on your documents matches the name on your passport.

Section 1: BASIC INFORMATION
Print all information (name, date, gender, etc.) neatly and legibly. Your birth date should be listed as the month first, then the day followed by the year.

Section 2: VISA INFORMATION
Please indicate which applies to you. If you are going to be applying for a J visa, please fill in the necessary blanks accurately. If you are in another visa status besides F or J and do not wish to change to an F or J visa status please do not complete this form. Please only fill out the Visa Clearance Form and return the form to OISSS with copies of your visa documents.

Section 3: FINANCIAL INFORMATION:
Please indicate how you will be supported in the space provided. Attach all necessary financial documents to this form. Do not send them in separately. Bank statements that are not attached to the CFR should not be sent -- OISSS processes more than 1,000 CFRs and we cannot "match" bank statements with CFRs. No action will be taken on unsolicited or separate bank statements. You must show at least $30,428 in support ($17,350 for tuition/fees, $10,800 for living expenses (9 months), $1478 for health insurance and $800 for books). This is an estimate; tuition and fees are subject to change. Please anticipate a 6-10% increase per year.
An applicant who wishes to support him/herself must show funds on deposit for the full-term of his/her program (including support of dependents, if any). Applicants who are being sponsored only have to show funding on deposit for 1 year. This is because it is assumed that sponsors continue to work while the student is enrolled in school. Thus, the sponsor is continually earning money. If a student is being supported by personal funds, his/her income while in school is negligible, thus the need to show all of the money up front, before starting the academic program.
An applicant who will be financially sponsored by his/her employer, home government or university, must provide an official letter of support from his/her scholarship or fellowship source with the current date and an original signature indicating the amount and duration of the award (2 years for a Master's degree and 4-5 for a Bachelor's or Doctoral degree). The letter should also include the applicant's name, academic program, and that it is valid specifically for attendance at North Carolina State University. A student who plans to remain in the United States during the summer will need approximately $3,000 for living expenses. You should not expect employment, either part-time or during summers, to be a significant means of support.

Section 4: SPONSOR INFORMATION:
If a sponsor (relative or friend) will provide financial support for the applicant, the sponsor must sign the appropriate signature line on the CFR and indicate continuing support for subsequent years. If necessary, the sponsor can write and sign a separate affidavit of support instead of completing this section. If the sponsor is in the US then he/she must be a US Citizen or Permanent Resident and must attach a copy of his/her resident alien card.

Section 5: FINANCIAL DOCUMENTATION:
Please have a bank official fill out this section of the form. The bank information must certify that the money is currently on deposit in liquid assets. General letters or statements from Chartered Accountants are not acceptable. Value assessments of properties or stocks/investments or solvency cannot be accepted. If funds are in a currency other than US Dollars, please be sure that the bank includes the current exchange rate or the equivalent amount in US dollars. Some banks will not agree to sign a document such as the CFR. If the bank is unwilling to complete this section, an alternative source of financial documentation must be provided. For example, the bank can submit a letter on bank letterhead detailing the amount on deposit or an original bank statement can be provided.

Section 6: DEPENDENT INFORMATION:
If you plan to bring your spouse or children with you, you must include their information here: Name, Date of Birth, Country of Birth Country of Citizenship, and Country of Permanent Residence. Please print information neatly and legibly. You will need to show an additional $6000 for your spouse and an extra $4000 for each additional child. If you have more dependents than the space provide, please attach an additional sheet of paper with their information provided.

Section 7: PERMANENT RESIDENTIAL ADDRESS IN HOME COUNTRY:
This information will be entered into SEVIS so make sure it is correct and legible. The address must be a street address and cannot include a Post Office Box number. It must be a physical address in order for us to issue your visa documents. It also must be your home address; this means it cannot be your place of work in the home country.

Section 8: IF IN US: CURRENT RESIDENTIAL ADDRESS IN THE US:
If you are currently inside the US, we must have your residential US address to enter the data into SEVIS. Please enter your home address. If a change occurs after you submit the information please e-mail Hanya_Redwan@ncsu.edu.

Section 9: OTHER INFORMATION:
Please make sure that the e-mail and telephone information is correct and current. If a CFR cannot be approved, the applicant will be notified via e-mail and provided with an explanation.

IMPORTANT!

The applicant must arrange to have two original sets of financial documents -- one attached to the CFR for OIS (or the Graduate School ONLY if applicant is overseas planning to apply for an initial F-1 visa) and one to present to the visa officer at the US consulate and immigration inspector at the US port of entry. The CFR is an internal form; that is, it is specific to NC State and OIS. The CFR will not be helpful in showing a visa officer at a US consulate overseas that the student has enough money.

Please retain a copy of the CFR and all documents submitted to OIS. Documents do get lost when traveling via international mail.
