March 27, 2008 Archived Version (Effective May 27, 2008 through June 9, 2008) Revised to clarify that originals of external letters received should be in the dossier sent forward and ultimately held in HR. Also revised Section 3-II-C to remove the inclusion of graduate students in the "mentoring activities" and Section 3-II-D to clarify that faculty are to include the graduate committees served in addition to those for which they are a major advisor.
June 10, 2008 Current Version - Deleted the last sentence in Section III-A of the Outline format for the dossier that reads: "Do not list submitted items, only include those that have been published or are in press for which a letter of acceptance is on file in the department". Upon request by the Chair of the Faculty in order to allow Faculty Senate and campus community consideration of this codification of practice, the Provost has agreed that this sentence be removed until that can occur.
