November 8, 2007 Archived Version (Effective November 8, 2007 through November 24, 2008) - Revisions are intended to ensure the University is in line with the Office of State Personnel (OSP) recommended appeal rights notice. Per OSP Guidelines: (1) Remove references to appeal rights on written warnings; (2) Remove appeal rights from probationary employees' disciplinary actions, including dismissals; (3) All other appeal rights remain the same. The effective date for the revisions is retroactive to December 1, 2006.

November 25, 2008 Archived Version (Effective November 25, 2008 through March 2, 2009) - Revised Section 1.2 to incorporate the Director of Employee Relations has the designee for the Associate Vice Chancellor for Human Resources to approve disciplinary actions.

March 3, 2009 Current Version - Minor revisions made under authority delegated to VC for Business and Finance. In section 10.4 deleted word “troubled”; changed EAP (Employee Assistance Program) to FASAP (Faculty and Staff Assistance Program).
