1. Purpose
This statement explains the procedures for requesting storage of assets
at off-campus locations which are privately owned or leased to the University.
2. General Guidelines
In order to maintain accurate inventory records and comply with asset control
procedures, the Capital Assets Office must be aware of and approve the storage
of equipment at off-campus locations. College/department personnel should
anticipate storage needs in advance in order to obtain the proper approvals
before removing the assets from the University.
3. Procedures
NCSU Form CA-3 is used to request storage of assets at locations that are
privately owned or leased to the University. The form must be signed by the
Dean/Director/Department Head and forwarded to the Capital Assets Office for
approval.

Form
CA-3, Storage of Equipment on Privately Owned/Leased Premises, may be
obtained from the Capital Assets Office, extension 5-2148. The form may be
copied for use by colleges/departments.
5. Instructions for Completion of Form
5.1. Storage of Equipment on Privately Owned/Leased Premises
5.1.1. Print name of Dean/Director/Department Head.
5.1.2. Enter location of off-campus storage.
5.1.3. Enter length of time item(s) will be stored.
5.1.4. State reason(s) for storing equipment off-campus.
5.1.5. Enter description of equipment and CAMS tag number(s).
5.1.6. Enter date of request.
5.1.7. Signature of Dean/Director/Department Head.
