1. Orders
1.1. Requests for adds, moves and changes to existing services should be sent to
the Communication Technologies Office, Customer Operations Area via Eform. To process the order the following information must be included:
- Department name
- Contact person name and number
- OUC, FAS and project if applicable
- Location where work is to take place
- Phone set type
1.2. If you do not receive a response within 3 business days after submitting your
order, you may request your services via email at customer_service@ncsu.edu

1.3. If your request involves more than 5 adds, moves or changes, the order should
be sent via email to customer_service@ncsu.edu.
1.4. Any adds, moves and changes must be done by the Communication Technologies
Office to ensure E911 records are kept accurate. The Communication Technologies
Office must also install all replacement telephone sets.
1.5. Once our office has received your request, a confirmation will be sent to the
individual requesting the service. The confirmation will contain an order number
and a general time frame in which work will be complete.
