1. Medical Eligibility for Trips - All participants will be required to fill out a "Trip Participant Medical/Health Statement" prior to each Outdoor Adventure Trip.

2. Rock wall Release Form - All patrons utilizing the Carmichael Gymnasium Rock wall will be required to fill out a "Release Form" and a "Rules and Regulations Form" or will not be able to utilize the wall.

3. Trip Forms- In addition to the "Medical Eligibility Form" see #1, Trip participants will be required to fill out an "Authorization for Emergency Medical Care" and a "Risks, Hazards, and Physical Stresses Release Form". Not filling out these forms will prevent patron from attending the trip.

4. Trip Cancellation Policy - A deposit is required for all participants to sign up for designated trip spaces. If cancellation occurs more than one week prior to the trip the deposit may be refunded less an administration fee. If cancellation occurs less than one week before the trip, no refund will be given.

5. Equipment Checkout - All Outdoor Adventure equipment is free of charge to current student, faculty, and staff with a valid All Campus Card. If a patron looses, or damages equipment they will be charged for replacement or repair of the item.
