1. Regulation:

Termination of care at Student Health Services will be done in a nondiscriminatory way and will be implemented only when there is just reason for such action.

2. Procedure:

2.1. Causes for termination include:

2.1.1. Patient repeatedly fails to make or keep appointments which result in negative clinical consequences.

2.1.2. Patient refuses to undergo recommended care or follow treatment plans, resulting in circumstances which jeopardize his/her health.

2.1.3. Patient engages in conduct which is grounds for disciplinary actions as determined by campus student conduct policies.

2.1.4. The Medical Director, Administrator, and other members of the Medical Staff, determine other cause for termination.

2.2. Exception to Rule:

Services will be provided for emergencies only or a referral made to a local hospital emergency department.

2.3. Action Taken:

Terminating services for a student is initiated only after staff conferences, involving the entire Medical Staff, fail to find an alternative means of resolving the problem.

2.4. Warning Procedures:

2.4.1. A letter is sent to the patient giving the reason(s) for such action and a warning that services at Student Health Services will be terminated if conduct continues. A return receipt is included to verify that the patient received the notice.

2.4.2. If the problem continues, a second letter is mailed to the student with a return receipt, notifying him/her that services will be terminated in 30 days. A reason will be given for the suspension. Names of local providers will be included along with a statement and a consent form (to be signed by the patient) allowing his/her medical records to be released to other health providers.

2.5. Grievance procedures:

If the patient feels that the termination was made in error or was unjustified, then he/she can seek advice from the Student Legal Services or go to a higher Administrative Officer in Student Affairs. Also reference the Grievance Procedure for Students Regulation
