When making payments for multiple members or if the credit card information is different from the application form, please indicate so in the PayPal Instructions Notes to Merchant at checkout.
Substitutions can be made up until May 14, 2011. If you are unable to attend but someone else from your organization can come in your place, or you are interested in transferring your registration to another, please notify us in writing via email at Kim.dittmann@nfoura.org. No refunds, cancellations, transfers or substitutions can be made after May 14, 2011.
Do not fax your receipt and registration if you have registered online.
If a third party is paying for your registration or if there are multiple registrations, please email Kim Dittmann Kim.Dittmann@nfoura.org, the name of the payor, and the corresponding registrants.
All registrations must be accompanied by FULL payment.
