Welcome to the STEM gaming survey. This survey is part of a research study about computer gaming and how it can be used in education. We are asking you to participate in this study by completing the following survey. The survey asks for information about you such as your age and where you’re from, and asks your opinions about computer gaming and education. It should only take about 5 minutes. Participation in this study is completely voluntary. You don’t have to complete the survey if you don’t want to.

There shouldn’t be any risks to you from the questions, but if you’re under 18 years old, you should ask you mom or dad if it’s okay for you to answer the questions.

While there will be no direct benefit to you from the study, we hope to learn more about how to make education more fun and effective.

Any information you give us on the survey will be anonymous. We won’t know who you are.

If you have questions about the research, you can contact Dr. Aaron Clark at 919-515-1771.

If you have any questions about your rights as a research subject, you can contact Mr. Matthew Ronning, Assistant Vice Chancellor, Research Administration, Box 7514, NCSU Campus (919/513-2148)
