Registering with the DSO

The registration process is the first move towards receiving services of any kind from the DSO. Following these steps will help to ensure that your registration is completed in a timely manner.

Step 1
Complete the DSO Student Application form.

Step 2
Complete the FERPA Consent to Release Information form.

Step 3
Submit your documentation. The DSO Service Provider will review the documentation and contact you (usually via email) to notify you of your eligibility status.

Step 4
Schedule an appointment with your DSO service provider to discuss services and/or accommmodations. This meeting is required for accommodations and/or services to begin.

Step 5
Meet with your DSO service provider to:
