If you would like for me to write a letter of recommendation for you, you must ask me for an information sheet no later than three weeks in advance of the due date for the recommendation. This information sheet must be completed and returned to me no later than two weeks in advance of the due date. Return this information sheet to me with forms from your college or employer. In addition, you must supply me with an addressed envelope for each letter I need to write. If the letter is to be mailed by me, the envelope must be stamped with the correct postage.

If the letter of recommendation is for NCSSM, your information sheet, forms and stamped envelope must be returned to me before the Thanksgiving holidays.
