Please read this important information before submitting your Health Form!
Student Health is currently accepting Health Forms for 2008 Summer and 2008 Fall sessions. It is important to turn in forms timely to avoid cancellation of classes and a reinstatement fee. Students under the age of 18 must have a parent or guardian sign the Health Form. Please pay particular attention to the following information.
Additional immunizations required for students enrolling in college for the first time after July 1, 2008:
1. Proof of two Measles, Mumps, Rubella vaccinations or laboratory proof of immunity
2. Proof of a Tdap booster within the past ten years (unless a Td booster was received
within the past ten years) in addition to the childhood series
*International students and non-U.S. citizens must have a tuberculin (PPD) test administered in a U.S. facility within the 12-month period preceding the first day of classes. TB test results must be stated in millimeters.
