Students diagnosed with ADD / ADHD
If you have already been formally diagnosed with ADD/ADHD by another physician, psychiatrist, or psychologist and wish to continue treatment with Student Health please read the following: At your first appointment with Student Health you need to have with you: Completed and signed copy of Request for Verification of ADD/ADHD Diagnosis from the diagnosing doctor. OR Copy of Formal ADD/ADHD Testing clearly stating diagnosis of ADD/ADHD. OR Office visits with physician or other provider clearly stating diagnosis and treatment plans. You will be required to see a provider each month to get your medication refills. We cannot “call in” medications for anyone. Charges for ADD/ADHD visits and medications: Visit cost is $20/month Medication costs vary Please call our appointment line at 515-7107 to schedule a “NEW ADD/ADHD” appointment. Appointments may be limited. Not all of our providers treat ADD/ADHD, therefore, we encourage you to call and come in well before you are OUT of medications.
