Please read the following important information before submitting your Immunization and Medical History Form:

North Carolina public law requires proof of immunizations to protect students while at NC State and also requires that students be dropped from classes if the immunizations requirements are not met. There is a $150 fee to re-enroll.

Students under the age of 18 must sign the form and must have a parent or guardian sign the form.

EXCEPTIONS to North Carolina regulations include:

1. Students classified as “lifelong,” “distance education,” or "unclassified;”
2. Students registered in off-campus courses only;
3. Students attending night or weekend classes only; or
4. Students taking a course load of four (4) credit hours or less and residing off-campus.

International students and non-U.S. citizens must have a tuberculin (PPD) test administered in a U.S. facility within the 12-month period preceding the first day of classes. TB test results must be stated in millimeters. (Nil or negative is not acceptable.)

All records must be translated into English.

Please submit your Immunization and Medical History Form within 30 days of your acceptance. Mail or fax your completed form to the address or fax number below.

PLEASE NOTE: You must also submit your Immunization Record online at HealthWeb.
