How
much time should I allow before calling to check the status of my
request?
Requests are often completed in a matter of days. We are given 30
days to fulfill requests. Please allow at least two weeks before
calling to check the status.
Need to meet a deadline? Please call 919.513.3278 and every effort
will be made to assist you.
Why might there be a delay in receiving my medical
records?
The most likely reason is that records are stored off-site for students
who attended NC State greater than two years prior to the date of
the request are located in off-site storage; additional time is
needed for retrieval. Another reason is that we did not receive
a valid authorization from you. Please use the authorization form
on our website.
Is
there a charge for copying my medical records?
There is no charge for providing copies of your immunization records.
There is no charge for sending your medical record to your physician
or provider. Currently, there is a $10.00 minimum fee for medical
records released to you. There is no charge for your immunization
records.
Will you send my records to me AND to my provider?
We will send your records to one location. We encourage you to keep
a copy of your immunization records.
Will my records be faxed or mailed?
This is your choice. We will either fax or mail records. We generally
do not fax records of more than 10 pages in length. Please be sure
to provide correct addresses and fax numbers.
What if I want to amend the information in my medical record?
A: You may request that certain information in the medical record
be amended. For details, please contact the Medical Records department.
