Authorization-- your signature and
pertinent information on the form NCSU requires for release of your
medical information, whether to yourself or to a third party. May
be downloaded from SHS' Web site and faxed, mailed or delivered
in person.
FERPA-- The Family Educational Rights
and Privacy Act; applies to any student who is or has been in attendance
at NCSU.
HIPAA-- Health Insurance Portability
and Accountability Act signed into law in 1996; protects patients

Medical record—a legal document; a record of
your visit to a health care provider which serves to document services
related to your health care, treatment, payment for services
PHI (protected health information)--generally,
“any individually identifiable information, oral or recorded
in any form or medium that is created by a health care provider,
health plan, public health authority, employer, life insurer, school
university, or health care clearinghouse” and “relates
to the past, present or future payment for the provision of health
care to an individual”
Provider-- clinical staff including
board-certified physicians and physician extenders such as licensed
nurse practitioners and physician assistants.
