PLEASE READ THIS IMPORTANT INFORMATION
BEFORE SUBMITTING YOUR HEALTH FORM

Student Health is currently accepting Health Forms for 2008 Summer and 2008 Fall sessions. It is important to turn in forms timely to avoid cancellation of classes and a reinstatement fee. Students under the age of 18 must have a parent or guardian sign the Health Form. Please pay particular attention to the following information.

Additional immunizations for students enrolling in college for the first time after July 1, 2008:

1. Proof of two Measles, Mumps, Rubella vaccinations or laboratory proof of immunity

2. Proof of a Tdap booster within the past ten years (unless a Td booster was received
within the past ten years) in addition to the childhood series.

International students and non-U.S. citizens must have a tuberculin (PPD) test administered in a U.S. facility within the 12 months preceding the first day of classes. TB test results must be stated in millimeters.

Students may bring completed Health Forms to Orientation. For your convenience, Student Health Services staff will collect and screen Health Forms during Orientation.

Important dates:
May 19 - 1st day of Summer I classes
July 8 - 1st day of Summer II classes
August 18, 19, 20, 22- Immunizations by appointment (919-515-7107) 7:45am-4:30pm
August 20 - 1st day of Fall Semester

Please call to arrange an appointment for review of your records if desired. Walk-ins are welcome, but there may be a wait. Voicemail messages will be returned within two business days.

Mailed documents are preferred. Scanned documents are accepted via email with proper identification. Please call to verify receipt of scanned documents.
