Description

Study Abroad Credit

To be completed by the student upon return from a Study Abroad experience.

USP Requirements Completion Form

This form must be completed by all students at the beginning of the semester in which they intend to graduate.

Honors Contract

For students interested in earning honors recognition (towards partial fulfillment of USP requirements) in a regular course by completing additional requirements arranged with the course instructor.

Request for Credit: Independent Study or Research Course University Scholars may receive up to 6 hours credit towards the 15-hour USP course requirement through successful completion of approved independent study or independent research courses [listed on the 2nd page of this form]. To receive credit towards the 15 hours of required USP coursework through completion of a 400-level independent study or independent research course a student must complete this form and submit it to the University Scholars Program office at the beginning of the semester in which the course is taken.

USP Application for high school seniors

USP Application for current NCSU freshmen, sophomores, and transfer students Note: First year students with a 3.5 cumulative grade point average (3.75 for students in CALS and Engineering) after their first semester at NC State are invited to join the USP mid-year and begin the program in the spring. Transfer student and upperclassmen applications will be considered on a case-by-case basis. Please contact Sean Cassidy, Associate Director, at (919) 513-3984 for more information.
