This form must be completed by all students at the beginning of the semester in which they intend to graduate.

Honors Contract |
For students interested in earning honors recognition (towards partial fulfillment of USP requirements) in a regular course by completing additional requirements arranged with the course instructor.

| Request for Credit: Independent Study or Research Course |
University Scholars may receive up to 6 hours credit towards the 15-hour USP course requirement through successful completion of approved independent study or independent research courses [listed on the 2nd page of this form]. To receive credit towards the 15 hours of required USP coursework through completion of a 400-level independent study or independent research course a student must complete this form and submit it to the University Scholars Program office at the beginning of the semester in which the course is taken. |

Note: First year students with a 3.5 cumulative grade point average (3.75 for students in CALS and Engineering) after their first semester at NC State are invited to join the USP mid-year and begin the program in the spring.
