Biological Use Authorization Form

Duplicate Part D for use with Biological Use Authorization Form

Investigators must obtain approval from the Institutional Biosafety Committee (IBC) of the following biological materials prior to the procurement of the materials necessary to initiate the project:

  • recombinant DNA in organisms,
  • creation of transgenic plants or animals,
  • human and other primate-derived substances (blood, body fluids, cell lines or tissues),
  • organisms and viruses infectious to humans, animals or plants (e.g. parasites, viruses, bacteria, fungi, prions,
  • biologically active agents (i.e. toxins, allergens, venoms) that may cause disease in other living organisms or cause significant impact to the environment or community.

Registration and approval procedures and forms are described in the Laboratory Biosafety Manual , Chapter 1: "Procedures Governing the Use of Biohazardous Agents."

Investigators must procure all of the biological materials listed above according to the PRR which dictates order through the MarketPlace online procurement process or submission of a purchase request for items not presently in the Marketplace system. All projects are valid for three years from the date of approval.
Recombinant DNA research covered under the NIH Guidelines requires the Principal Investigator to classify which section of the NIH Guidelines applies.  For guidance, refer to the  Questionnaire for Classes of Experiments Covered Under NIH Guidelines.

Checklist for Biosafety Level 2 (BSL-2) at NC State University:

To ensure laboratories meet basic requirements at the federal, state, and local levels for BSL-2 practices and containment, the BSL-2 checklist should be completed and kept with the laboratory's Safety Plan.
